8e1034a526 xen/pci-swiotlb: reduce visibility of symbols
xen_swiotlb and pci_xen_swiotlb_init() are only used within the file
defining them, so make them static and remove the stubs. Otoh
pci_xen_swiotlb_detect() has a use (as function pointer) from the main
pci-swiotlb.c file - convert its stub to a #define to NULL.

a98f565462 xen-swiotlb: split xen_swiotlb_init
Split xen_swiotlb_init into a normal an an early case.  That makes both
much simpler and more readable, and also allows marking the early
code as __init and x86-only.

0a0f0d8be7 dma-mapping: split <linux/dma-mapping.h>
Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers.  That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

dceb1a6819 xen-swiotlb: consolidate xen_swiotlb_dma_ops
ARM and x86 had duplicated versions of the dma_ops structure, the
only difference is that x86 hasn't wired up the set_dma_mask,
mmap, and get_sgtable ops yet.  On x86 all of them are identical
to the generic version, so they aren't needed but harmless.

All the symbols used only for xen_swiotlb_dma_ops can now be marked
static as well.

7641842164 swiotlb-xen: Enforce return of DMA_ERROR_CODE in mapping function
The mapping function should always return DMA_ERROR_CODE when a mapping has
failed as this is what the DMA API expects when a DMA error has occurred.
The current function for mapping a page in Xen was returning either
DMA_ERROR_CODE or 0 depending on where it failed.

On x86 DMA_ERROR_CODE is 0, but on other architectures such as ARM it is
~0. We need to make sure we return the same error value if either the
mapping failed or the device is not capable of accessing the mapping.

If we are returning DMA_ERROR_CODE as our error value we can drop the
function for checking the error code as the default is to compare the
return value against DMA_ERROR_CODE if no function is defined.

b827760053 xen/swiotlb: Use the swiotlb_late_init_with_tbl to init Xen-SWIOTLB late when PV PCI is used.
With this patch we provide the functionality to initialize the
Xen-SWIOTLB late in the bootup cycle - specifically for
Xen PCI-frontend. We still will work if the user had
supplied 'iommu=soft' on the Linux command line.

Note: We cannot depend on after_bootmem to automatically
determine whether this is early or not. This is because
when PCI IOMMUs are initialized it is after after_bootmem but
before a lot of "other" subsystems are initialized.

b8b0f559c7 xen/apic/xenbus/swiotlb/pcifront/grant/tmem: Make functions or variables static.
There is no need for those functions/variables to be visible. Make them
static and also fix the compile warnings of this sort:

drivers/xen/<some file>.c: warning: symbol '<blah>' was not declared. Should it be static?

Some of them just require including the header file that
declares the functions.

fc2341df9e xen/swiotlb: With more than 4GB on 64-bit, disable the native SWIOTLB.
If a PV guest is booted the native SWIOTLB should not be
turned on. It does not help us (we don't have any PCI devices)
and it eats 64MB of good memory. In the case of PV guests
with PCI devices we need the Xen-SWIOTLB one.

988f0e24bb xen/swiotlb: Simplify the logic.
Its pretty easy:
 1). We only check to see if we need Xen SWIOTLB for PV guests.
 2). If swiotlb=force or iommu=soft is set, then Xen SWIOTLB will
     be enabled.
 3). If it is an initial domain, then Xen SWIOTLB will be enabled.
 4). Native SWIOTLB must be disabled for PV guests.

5cb3a26793 x86, xen-swiotlb: Make Xen-SWIOTLB use IOMMU_INIT_* macros.
We utilize the IOMMU_INIT macros to create this dependency:

       [pci_xen_swiotlb_detect]
                 |
       [pci_swiotlb_detect_override]
                 |
       [pci_swiotlb_detect_4gb]

In other words, we set 'pci_xen_swiotlb_detect' to be
the first detection to be run during start.

bbbe57386e pci-swiotlb-xen: Add glue code to setup dma_ops utilizing xen_swiotlb_*
functions.

We add the glue code that sets up a dma_ops structure with the
xen_swiotlb_* functions. The code turns on xen_swiotlb flag
when it detects it is running under Xen and it is either
in privileged mode or the iommu=soft flag was passed in.

It also disables the bare-metal SWIOTLB if the Xen-SWIOTLB has
been enabled.

Note: The Xen-SWIOTLB is only built when CONFIG_XEN is enabled.
