In case of error, the function of_phy_connect() returns NULL
pointer not ERR_PTR(). The IS_ERR() test in the return value
check should be replaced with NULL test.

The device tree will supply the register bank base addresses, make
register addressing relative to those. PHY connection is now
described by the device tree.
The OCTEON_IRQ_MII{0,1} symbols are also removed as they are now
unused and interfere with the irq_domain used for device tree irq
mapping.

Commit "d6c25be: mdio-octeon: use an unique MDIO bus name" changed the
octeon MDIO bus name from "0" to "mdio-octeon-0", change the PHY
formatting logic to account for that name change, so that PHY connection
on this bus succeeds.

The device.h header was including module.h, making it present for
most of these drivers. But we want to clean that up. Call out the
include of module.h in the modular network drivers.

Use IFF_UNICAST_FTL to find out if driver handles unicast address
filtering. In case it does not, promisc mode is entered.
Patch also fixes following drivers:
stmmac, niu: support uc filtering and yet it propagated
ndo_set_multicast_list
bna, benet, pxa168_eth, ks8851, ks8851_mll, ksz884x : has set
ndo_set_rx_mode but do not support uc filtering
