1821438a9b [SCSI] don't reap targets upon device_add failure
This patch (as1358) fixes a bug in the error pathway of
scsi_target_add().  If registration fails, the target should not be
reaped.  The reaping occurs later, when scanning is finished and all
the child devices are removed.  The current code leaves an unbalanced
value in starget->reap_ref.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

4cb077d93a PM: Allow SCSI devices to suspend/resume asynchronously
Set power.async_suspend for all SCSI devices, targets and hosts, so
that they can be suspended and resumed in parallel with the main
suspend/resume thread and possibly with other devices they don't
depend on in a known way (i.e. devices which are not their parents or
children).

The power.async_suspend flag is also set for devices that don't have
suspend or resume callbacks, because otherwise they would make the
main suspend/resume thread wait for their "asynchronous" children
(during suspend) or parents (during resume), effectively negating the
possible gains from executing these devices' suspend and resume
callbacks asynchronously.

This patch (as1335) fixes a bug in scsi_sysfs_add_sdev().  Its callers
always remove the device if anything goes wrong, so it should never
remove the device.

4a84067dbf [SCSI] add queue_depth ramp up code
Current FC HBA queue_depth ramp up code depends on last queue
full time. The sdev already  has last_queue_full_time field to
track last queue full time but stored value is truncated by
last four bits.

So this patch updates last_queue_full_time without truncating
last 4 bits to store full value and then updates its only
current usages in scsi_track_queue_full to ignore last four bits
to keep current usages same while also use this field
in added ramp up code.

Adds scsi_handle_queue_ramp_up to ramp up queue_depth on
successful completion of IO. The scsi_handle_queue_ramp_up will
do ramp up on all luns of a target, just same as ramp down done
on all luns on a target.

The ramp up is skipped in case the change_queue_depth is not
supported by LLD or already reached to added max_queue_depth.

Updates added max_queue_depth on every new update to default
queue_depth value.

The ramp up is also skipped if lapsed time since either last
queue ramp up or down is less than LLD specified
queue_ramp_up_period.

Adds queue_ramp_up_period to sysfs but only if change_queue_depth
is supported since ramp up and queue_ramp_up_period is needed only
in case change_queue_depth is supported first.

Initializes queue_ramp_up_period to 120HZ jiffies as initial
default value, it is same as used in existing lpfc and qla2xxx.

 Moves max_queue_depth initialization after slave_configure is
called from after slave_alloc calling done. Also adjusted
max_queue_depth check to skip ramp up if current queue_depth
is >= max_queue_depth.

to store or show its value.

e881a172da [SCSI] modify change_queue_depth to take in reason why it is being called
This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.

This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

860dc73608 [SCSI] fix async scan add/remove race resulting in an oops
Async scanning introduced a very wide window where the SCSI device is
up and running but has not yet been added to sysfs.  We delay the
adding until all scans have completed to retain the same ordering as
sync scanning.

This delay in visibility causes an oops if a device is removed before
we make it visible because the SCSI removal routines have an inbuilt
assumption that if a device is in SDEV_RUNNING state, it must be
visible (which is not necessarily true in the async scanning case).

Fix this by introducing an additional is_visible flag which we can use
to condition the tear down so we do the right thing for running but
not yet made visible.

37e6ba0072 [SCSI] fix memory leak in initialization
The root cause of the problem is the fact that dev_set_name() now
allocates storage instead of using the original array within the kobj.
That means that the SCSI assumption that if you haven't made the
containing object or any sub objects visible, you can just destroy it
(and its component devices) lock stock and barrel becomes false.

Fix this by doing the get of sdev_dev at parent time and thus do an
extra put of it in scsi_destroy_sdev() (and all other destruction
without add paths).

b391277a56 sd, sr: fix Driver 'sd' needs updating message
If a SCSI ULD driver sets blk_queue_prep_rq(), it should clean it
up itself on remove(), and not from the bus callbacks. This
removes the need to hook into bus->remove(), which should not
be used at the same time as driver->remove().

242f9dcb8b block: unify request timeout handling
Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.

Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

6f4267e3bd [SCSI] Update the SCSI state model to allow blocking in the created state
Brian King <brking@linux.vnet.ibm.com> reported that fibre channel
devices can oops during scanning if their ports block (because the
device goes from CREATED -> BLOCK -> RUNNING rather than CREATED ->
BLOCK -> CREATED).

Fix this by adding a new state: CREATED_BLOCK which can only transition
back to CREATED and disallow the CREATED -> BLOCK transition.  Now both
the created and blocked states that the mid-layer recognises can include
CREATED_BLOCK.

4469f98780 [SCSI] Host protection capabilities
Controllers that support protection information must indicate this to
the SCSI midlayer so that the ULD can prepare scsi_cmnds accordingly.

This patch implements a host mask and various types of protection:

 - DIF Type 1-3 (between HBA and disk)
 - DIX Type 0-3 (between OS and HBA)

The patch also allows the HBA to set the guard type to something
different than the T10-mandated CRC.

a6a8d9f87e [SCSI] scsi_dh: add infrastructure for SCSI Device Handlers
Some of the storage devices (that can be accessed through multiple paths),
do need some special handling for
	1. Activating the passive path of the storage access.
	2. Decode and handle the special sense codes returned by the devices.
	3. Handle the I/Os being sent to the passive path, especially
           during the device probe time.
when accessed through multiple paths.

As of today this special device handling is done at the dm-multipath
layer using dm-handlers. That works well for (1); for (2) to be handled
at dm layer, scsi sense information need to be exported from SCSI to dm-layer,
which is not very attractive; (3) cannot be done at all at the dm layer.

Device handler has been moved to SCSI mainly to handle (2) and (3) properly.

which added scsi_bus_type to the struct scsi_target device.  This
causes both the scsi_device and scsi_target to fire scsi_bus_uevents.
However, the actualy scsi_bus_uevent() call assumes blindly that it's
a struct scsi_device.  Check for this and return immediately if it
isn't.

97f46ae45c [SCSI] bsg: add release callback support
This patch adds release callback support, which is called when a bsg
device goes away. bsg_register_queue() takes a pointer to a callback
function. This feature is useful for stuff like sas_host that can't
use the release callback in struct device.

If a caller doesn't need bsg's release callback, it can call
bsg_register_queue() with NULL pointer (e.g. scsi devices can use
release callback in struct device so they don't need bsg's callback).

With this patch, bsg uses kref for refcounts on bsg devices instead of
get/put_device in fops->open/release. bsg calls put_device and the
caller's release callback (if it was registered) in kref_put's
release.

643eb2d932 [SCSI] rework scsi_target allocation
The current target allocation code registeres each possible target
with sysfs; it will be deleted again if no useable LUN on this target
was found. This results in a string of 'target add/target remove' uevents.

Based on a patch by Hannes Reinecke <hare@suse.de> this patch reworks
the target allocation code so that only uevents for existing targets
are sent. The sysfs registration is split off from the existing
scsi_target_alloc() into a in a new scsi_add_target() function, which
should be called whenever an existing target is found. Only then a
uevent is sent, so we'll be generating events for existing targets
only.

d52b3815a5 [SCSI] add missing transport configure points for target and host
While trying to convert the SPI transport class to attribute groups, I
discovered that we don't actually have any transport configure points
for either the target or the host.  This patch adds these missing
transport class triggers.  The host one is simply done after the add,
the target one tries to be more clever and add it after devices may have
been placed on the target (so the device configure will have set up the
target parameters).

751bf4d786 [SCSI] scsi_sysfs: restore prep_fn when ULD is removed
A recent bug report:

http://bugzilla.kernel.org/show_bug.cgi?id=9674

Was caused because the ULDs now set their own prep functions, but
don't necessarily reset the prep function back to the SCSI default
when they are removed.  This leads to panics if commands are sent to
the device after the module is removed because the prep_fn is still
pointing to the old module code.  The fix for this is to implement a
bus remove method that resets the prep_fn pointer correctly before
calling the ULD specific driver remove method.

7eff2e7a8b Driver core: change add_uevent_var to use a struct
This changes the uevent buffer functions to use a struct instead of a
long list of parameters. It does no longer require the caller to do the
proper buffer termination and size accounting, which is currently wrong
in some places. It fixes a known bug where parts of the uevent
environment are overwritten because of wrong index calculations.

Many thanks to Mathieu Desnoyers for finding bugs and improving the
error handling.

bfd129445f [SCSI] switch sdev sysfs attributes to default attributes
This removes the unused sysfs attribute overwriting logic for most of
the attributes, and plugs them into the driver core default attribute
creation.

Without this patch, at the time of the events for the SCSI LUN's, there
will be no sysfs files, because their creation is delayed until the sd
driver has spun up the disks, which might take several seconds. It is the
last WAIT_FOR_SYSFS rule in the default udev setup which can be removed
with this change.

5dc2b89e12 [SCSI] add supported_mode and active_mode attributes to the host
This adds supported_mode and active_mode attributes to
/sys/class/sys_host/hostX/ for specifying the mode that a lld supports
and the currently activated mode. The output format is similar to fc
rport roles:

luce:/sys/class/scsi_host/host0$ cat supported_mode
Initiator
luce:/sys/class/scsi_host/host0$ cat active_mode
Initiator

The mode values uses bitmap since we would support dual-mode llds in
the future like this:

luce:/sys/class/scsi_host/host0$ cat supported_mode
Initiator, Target

The supported_mode attribute looks at a scsi_host_template and the
active_mode attribute looks at a scsi_host. We would add a hook to a
scsi_host_template to change the active_mode attribute
dynamically. But now there is no hook since no lld supports that
feature.

39dca558a5 [SCSI] bsg: make class backlinks
Currently, bsg doesn't make class backlinks (a process whereby you'd get
a link to bsg in the device directory in the same way you get one for
sg).  This is because the bsg device is uninitialised, so the class
device has nothing it can attach to.  The fix is to make the bsg device
point to the cdevice of the entity creating the bsg, necessitating
changing the bsg_register_queue() prototype into a form that takes the
generic device.

80ed71ce1a [SCSI] bsg: separate bsg and SCSI (so SCSI can be modular)
This patch moves the bsg registration into SCSI so that bsg no longer
has a dependency on the scsi_interface_register API.

This can be viewed as a temporary expedient until we can get universal
bsg binding sorted out properly.  Also use the sdev bus_id as the
generic bsg name (to avoid clashes with the queue name).

1dfcda06a6 [SCSI] kill scsi host template suspend/resume
With libata converted to use sdev->manage_start_stop for suspend and
resume, sht->suspend/resume() has no user left and low level
suspend/ressume should be taken care of by low level driver's
suspend/resume callbacks (e.g. PCI or PCMCIA driver callbacks).  This
patch removes sht->suspend/resume() callbacks.

This change is suggested by Christoph Hellwig.

The following patch adds support for sysfs/uevent modalias
attribute for scsi devices (like disks, tapes, cdroms etc),
based on whatever current sd.c, sr.c, st.c and osst.c drivers
supports.

The modalias format is like this:

 scsi:type-0x04

(for TYPE_WORM, handled by sr.c now).

Several comments.

o This hexadecimal type value is because all TYPE_XXX constants
  in include/scsi/scsi.h are given in hex, but __stringify() will
  not convert them to decimal (so it will NOT be scsi:type-4).
  Since it does not really matter in which format it is, while
  both modalias in module and modalias attribute match each other,
  I descided to go for that 0x%02x format (and added a comment in
  include/scsi/scsi.h to keep them that way), instead of changing
  them all to decimal.

o There was no .uevent routine for SCSI bus.  It might be a good
  idea to add some more ueven environment variables in there.

o osst.c driver handles tapes too, like st.c, but only SOME tapes.
  With this setup, hotplug scripts (or whatever is used by the
  user) will try to load both st and osst modules for all SCSI
  tapes found, because both modules have scsi:type-0x01 alias).
  It is not harmful, but one extra module is no good either.
  It is possible to solve this, by exporting more info in
  modalias attribute, including vendor and device identification
  strings, so that modalias becomes something like
    scsi:type-0x12:vendor-Adaptec LTD:device-OnStream Tape Drive
  and having that, match for all 3 attributes, not only device
  type.  But oh well, vendor and device strings may be large,
  and they do contain spaces and whatnot.
  So I left them for now, awaiting for comments first.

c3c94c5a2f [SCSI] sd: implement START/STOP management
Implement SBC START/STOP management.  sdev->mange_start_stop is added.
When it's set to one, sd STOPs the device on suspend and shutdown and
STARTs it on resume.  sdev->manage_start_stop defaults is in sdev
instead of scsi_disk cdev to allow ->slave_config() override the
default configuration but is exported under scsi_disk sysfs node as
sdev->allow_restart is.

When manage_start_stop is zero (the default value), this patch doesn't
introduce any behavior change.

d9a9cdfb07 [PATCH] sysfs and driver core: add callback helper, used by SCSI and S390
This patch (as868) adds a helper routine for device drivers that need
to set up a callback to perform some action in a different process's
context.  This is intended for use by attribute methods that want to
unregister themselves or their parent device.  Attribute method calls
are mutually exclusive with unregistration, so such actions cannot be
taken directly.

Two attribute methods are converted to use the new helper routine: one
for SCSI device deletion and one for System/390 ccwgroup devices.

7c9d6f16f5 [SCSI] SCSI core: better initialization for sdev->scsi_level
This patch will affect the CDB in INQUIRY commands sent to LUNs above 0 
when LUN-0 reports a scsi_level of 0; the LUN bits will no longer be set 
in the second byte of the CDB.  This is as it should be.  Nevertheless, 
it's possible that some wacky device might be adversely affected.  I doubt 
anyone will complain...

65f27f3844 WorkStruct: Pass the work_struct pointer instead of context data
Pass the work_struct pointer to the work function rather than context data.
The work function can use container_of() to work out the data.

For the cases where the container of the work_struct may go away the moment the
pending bit is cleared, it is made possible to defer the release of the
structure by deferring the clearing of the pending bit.

To make this work, an extra flag is introduced into the management side of the
work_struct.  This governs auto-release of the structure upon execution.

Ordinarily, the work queue executor would release the work_struct for further
scheduling or deallocation by clearing the pending bit prior to jumping to the
work function.  This means that, unless the driver makes some guarantee itself
that the work_struct won't go away, the work function may not access anything
else in the work_struct or its container lest they be deallocated..  This is a
problem if the auxiliary data is taken away (as done by the last patch).

However, if the pending bit is *not* cleared before jumping to the work
function, then the work function *may* access the work_struct and its container
with no problems.  But then the work function must itself release the
work_struct by calling work_release().

In most cases, automatic release is fine, so this is the default.  Special
initiators exist for the non-auto-release case (ending in _NAR).

082776e4be [PATCH] Make libata not powerdown drivers on PM_EVENT_FREEZE.
At the moment libata doesn't pass pm_message_t down ata_device_suspend.
This causes drives to be powered down when we just want a freeze,
causing unnecessary wear and tear. This patch gets pm_message_t passed
down so that it can be used to determine whether to power down the
drive.
