b2bbb20b37 USB: pxa2xx_udc understands GPIO based VBUS sensing
This updates the PXA 25x UDC board-independent infrastructure for VBUS sensing
and the D+ pullup. The original code evolved from rather bizarre support on
Intel's "Lubbock" reference hardware, so that on more sensible hardware it
doesn't work as well as it could/should.

The change is just to teach the UDC driver how to use built-in PXA GPIO pins
directly. This reduces the amount of board-specfic object code needed, and
enables the use of a VBUS sensing IRQ on boards (like Gumstix) that have one.
With VBUS sensing, the UDC is unclocked until a host is actually connected.

f12d0d7c77 [ARM] nommu: manage the CP15 things
All the current CP15 access codes in ARM arch can be categorized and
conditioned by the defines as follows:

     Related operation	Safe condition
  a. any CP15 access	!CPU_CP15
  b. alignment trap	CPU_CP15_MMU
  c. D-cache(C-bit)	CPU_CP15
  d. I-cache		CPU_CP15 && !( CPU_ARM610 || CPU_ARM710 ||
				CPU_ARM720 || CPU_ARM740 ||
				CPU_XSCALE || CPU_XSC3 )
  e. alternate vector	CPU_CP15 && !CPU_ARM740
  f. TTB CPU_CP15_MMU
  g. Domain CPU_CP15_MMU
  h. FSR/FAR CPU_CP15_MMU

For example, alternate vector is supported if and only if
"CPU_CP15 && !CPU_ARM740" is satisfied.

fefdaa06cc [ARM] nommu: defines CPU_CP15, CPU_CP15_MMU and CPU_CP15_MPU
By merging of uClinux/ARM, we need to treat various CPU cores which have
MMU, MPU or even none for memory management. The memory management
coprocessors are controlled by CP15 register set and the ARM core family
can be categorized by 5 groups by the register ;
  G-a. CP15 is MMU : 610, 710, 720, 920, 922, 925, 926, 1020, 1020e, 1022,
		v6 and the derivations sa1100, sa110, xscale, xsc3.
  G-b. CP15 is MPU : 740, 940, 946, 996, 1156.
G-c. CP15 is MPU or MMU : 1026 (selectable by schematic design)
  G-d. CP15 is exist, but nothing for memory managemnt : 966, 968.
  G-e. no-CP15 : 7tdmi, 9tdmi, 9e, 9ej

This patch defines CPU_CP15, CPU_CP15_MMU and CPU_CP15_MPU. Thus the
family can be defined as :
  - CPU_CP15 only : G-d
  - CPU_CP15_MMU(implies CPU_CP15) : G-a, G-c(selectable)
  - CPU_CP15_MPU(implies CPU_CP15) : G-b, G-c(selectable)
  - !CPU_CP15 : G-e

4052ebb7a2 [ARM] 3859/1: Fix devicemaps_init() XIP_KERNEL odd 1MiB XIP_PHYS_ADDR translation error
The ARM XIP_KERNEL map created in devicemaps_init() is wrong.
The map.pfn is rounded down to an even 1MiB section boundary

46a82b2d55 [PATCH] Standardize pxx_page macros
One of the changes necessary for shared page tables is to standardize the
pxx_page macros. pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address. pud_page and pgd_page, on the
other hand, return the kernel virtual address.

Shared page tables needs pud_page and pgd_page to return the actual page
structures. There are very few actual users of these functions, so it is
simple to standardize their usage.

Since this is basic cleanup, I am submitting these changes as a standalone
patch. Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

197c9444d6 [ARM] 3814/1: move 80200 dma_inv_range() erratum check out of line
On stepping A0/A1 of the 80200, invalidating D-cache by line doesn't
clear the dirty bits, which means that if we invalidate a dirty line,
the dirty data can still be written back to memory later on.

To work around this, dma_inv_range() on these two processors is
implemented as dma_flush_range() (i.e. do a clean D-cache line before
doing the invalidate D-cache line.) For this, we currently have a
processor ID check in xscale_dma_inv_range(), but a better solution
is to add a separate cache_fns and proc_info for A0/A1 80200.

6a39dd6222 [ARM] 3759/2: Remove uses of %?
Patch from Daniel Jacobowitz

The ARM kernel has several uses of asm("foo%?"). %? is a GCC internal
modifier used to output conditional execution predicates. However, no
version of GCC supports conditionalizing asm statements. GCC 4.2 will
correctly expand %? to the empty string in user asms. Earlier versions may
reuse the condition from the previous instruction. In 'if (foo) asm
("bar%?");' this is somewhat likely to be right... but not reliable.

So, the only safe thing to do is to remove the uses of %?. I believe
the tlbflush.h occurances were supposed to be removed before, based
on the comment about %? not working at the top of that file.

Old versions of GCC could omit branches around user asms if the asm didn't
mark the condition codes as clobbered. This problem hasn't been seen on any
recent (3.x or 4.x) GCC, but it could theoretically happen. So, where
%? was removed a cc clobber was added.

a188ad2bc7 [ARM] 3762/1: Fix ptrace cache coherency bug for ARM1136 VIPT nonaliasing Harvard caches
Patch from George G. Davis

Resolve ARM1136 VIPT non-aliasing cache coherency issues observed when
using ptrace to set breakpoints and cleanup copy_{to,from}_user_page()
while we're here as requested by Russell King because "it's also far
too heavy on non-v6 CPUs".

NOTES:

1. Only access_process_vm() calls copy_{to,from}_user_page().
2. access_process_vm() calls get_user_pages() to pin down the "page".
3. get_user_pages() calls flush_dcache_page(page) which ensures cache
coherency between kernel and userspace mappings of "page". However
flush_dcache_page(page) may not invalidate I-Cache over this range
for all cases, specifically, I-Cache is not invalidated for the VIPT
non-aliasing case. So memory is consistent between kernel and user
space mappings of "page" but I-Cache may still be hot over this
range. IOW, we don't have to worry about flush_cache_page() before
   memcpy().
4. Now, for the copy_to_user_page() case, after memcpy(), we must flush
the caches so memory is consistent with kernel cache entries and
invalidate the I-Cache if this mm region is executable. We don't
need to do anything after memcpy() for the copy_from_user_page()
case since kernel cache entries will be invalidated via the same
process above if we access "page" again. The flush_ptrace_access()
function (borrowed from SPARC64 implementation) is added to handle
cache flushing after memcpy() for the copy_to_user_page() case.

cea6a4ba8a [PATCH] panic_on_oops: remove ssleep()
This patch is part of an effort to unify the panic_on_oops behaviour across
all architectures that implement it.

It was pointed out to me by Andi Kleen that if an oops has occured in
interrupt context, then calling sleep() in the oops path will only cause a
panic, and that it would be really better for it not to be in the path at
all.

This patch removes the ssleep() call and reworks the console message
accordinly. I have a slght concern that the resulting console message is
too long, feedback welcome.

For powerpc it also unifies the 32bit and 64bit behaviour.

Fror x86_64, this patch only updates the console message, as ssleep() is
already not present.

7b1df1985c [ARM] 3737/1: Export ARM copy/clear_user_page symbols
Patch from George G. Davis

As reported by various folks on the ARM Linux kernel mailing list,
the video-buf.ko driver has undefined references on all ARM machines
which use it as observed during `make modules`:

Warning: "v4wb_clear_user_page" [drivers/media/video/video-buf.ko] undefined!

Similar warnings exist for all ARM machines which use this driver.
So this change adds the missing EXPORT_SYMBOLs to allow using this
driver as a module.

a6a38a6622 [ARM] 3736/1: xscale: don't mis-report 80219 as an iop32x
Patch from Lennert Buytenhek

The IOP 80219 xscale CPU is a stripped down version of the IOP32x.
But the fact that the 80219 and IOP32x are very similar doesn't mean
that they need to share a cpu table entry. It's also somewhat confusing
for the end user to see the 80219 reported as an IOP32x, so this patch
splits the IOP32x cpu table entry to make a separate entry for the
80219.

screen_info.h doesn't have anything to do with the tty layer and shouldn't be
included by tty.h. This patches removes the include and modifies all users to
directly include screen_info.h. struct screen_info is mainly used to
communicate with the console drivers in drivers/video/console. Note that this
patch touches every arch and I have no way of testing it. If there is a
mistake the worst thing that will happen is a compile error.

4a2581a080 [ARM] 3692/1: ARM: coswitch irq handling to the generic implementation
Patch from Thomas Gleixner

From: Thomas Gleixner <tglx@linutronix.de>

Switch the ARM irq core handling to the generic implementation. The
ARM specific header files now contain mostly migration stubs and
helper macros. Note that each machine type must be converted after
this step seperately. This was seperated out from the patch for easier
review.

The main changes for the machine type code is the conversion of the
type handlers to a 'type flow' and 'chip' model. This affects only the
multiplex interrupt handlers. A conversion macro needs to be added to
those implementations, which defines the data structure which is
registered by the set_irq_chained_handler() macro.

Some minor fixups of include files and the conversion of data
structure access is necessary all over the place.

The mostly macro based conversion was provided to allow an easy
migration of the existing implementations.

The code compiles on all defconfigs available in arch/arm/configs
except those which were broken also before applying the conversion
patches.

The code has been boot and runtime tested on most ARM platforms. The
results of an extensive testing and bugfixing series can be found

8799ee9f49 [ARM] Set bit 4 on section mappings correctly depending on CPU
On some CPUs, bit 4 of section mappings means "update the
cache when written to". On others, this bit is required to
be one, and others it's required to be zero. Finally, on
ARMv6 and above, setting it turns on "no execute" and prevents
speculative prefetches.

With all these combinations, no one value fits all CPUs, so we
have to pick a value depending on the CPU type, and the area
we're mapping.

22b1908610 [ARM] nommu: provide a way for correct control register value selection
Most MMU-based CPUs have a restriction on the setting of the data cache
enable and mmu enable bits in the control register, whereby if the data
cache is enabled, the MMU must also be enabled. Enabling the data
cache without the MMU is an invalid combination.

However, there are CPUs where the data cache can be enabled without the
MMU.

In order to allow these CPUs to take advantage of that, provide a
method whereby each proc-*.S file defines the control regsiter value
for use with nommu (with the MMU disabled.) Later on, when we add
support for enabling the MMU on these devices, we can adjust the
"crval" macro to also enable the data cache for nommu.

9641c7cc5a [ARM] nommu: uaccess tweaks
MMUless systems have only one address space for all threads, so
both the usual access_ok() checks, and the exception handling do
not make much sense.

Hence, discard the fixup and exception tables at link time, use
memcpy/memset for the user copy/clearing functions, and define
the permission check macros to be constants.

Some of this patch was derived from the equivalent patch by
Hyok S. Choi.

Patch from Guennadi Liakhovetski

Currently probe_irq_on() on PXA will silently reconfigure all output GPIOs, that are not configured as alternate functions, for input. Avoid that. Upon CPU reset all GPIOs are configured as inputs, so, if a GPIO is configured as output, it has been done so intentionally.

76b67ed9dc [PATCH] node hotplug: register cpu: remove node struct
With Goto-san's patch, we can add new pgdat/node at runtime. I'm now
considering node-hot-add with cpu + memory on ACPI.

I found acpi container, which describes node, could evaluate cpu before
memory. This means cpu-hot-add occurs before memory hot add.

In most part, cpu-hot-add doesn't depend on node hot add. But register_cpu(),
which creates symbolic link from node to cpu, requires that node should be
onlined before register_cpu(). When a node is onlined, its pgdat should be
there.

This patch-set holds off creating symbolic link from node to cpu
until node is onlined.

This removes node arguments from register_cpu().

Now, register_cpu() requires 'struct node' as its argument. But the array of
struct node is now unified in driver/base/node.c now (By Goto's node hotplug
patch). We can get struct node in generic way. So, this argument is not
necessary now.

This patch also guarantees add cpu under node only when node is onlined. It
is necessary for node-hot-add vs. cpu-hot-add patch following this.

Moreover, register_cpu calculates cpu->node_id by cpu_to_node() without regard
to its 'struct node *root' argument. This patch removes it.

Also modify callers of register_cpu()/unregister_cpu, whose args are changed
by register-cpu-remove-node-struct patch.

ea6dedd7fb ARM: OMAP: GPIO IRQ lazy IRQ disable fix
- The current OMAP GPIO IRQ framework doesn't use the do_edge_IRQ,
do_level_IRQ handlers, but instead calls do_simple_IRQ. This
doesn't handle disabled interrupts properly, so drivers will
still get interrupts after calling disable_irq. The patch solves
this by respecting the irq_desc.disable_depth and irq_desc.running
counters. When one of these is non-zero the handler is not called,
the interrupt is masked and marked as pending. The pending interrupt
will be serviced when the running handler returns. This is according
to the same semantics as the standard do_edge_IRQ and do_level_IRQ
handlers have, so one day we should use them instead of do_simple_IRQ.

- Process only interrupts that are not masked. The ISR may contain
pending interrupts that are masked these shouldn't be processed.

- Move the bank IRQ unmasking out of the IRQ dispatch loop. If there
are further iterations we shouldn't unmask it if there are level
  triggered interrupts pending.

68402ddc67 [PATCH] mm: remove VM_LOCKED before remap_pfn_range and drop VM_SHM
Remove VM_LOCKED before remap_pfn range from device drivers and get rid of
VM_SHM.

remap_pfn_range() already sets VM_IO. There is no need to set VM_SHM since
it does nothing. VM_LOCKED is of no use since the remap_pfn_range does not
place pages on the LRU. The pages are therefore never subject to swap
anyways. Remove all the vm_flags settings before calling remap_pfn_range.

After removing all the vm_flag settings no use of VM_SHM is left. Drop it.

1a6be26d5b [ARM] Enable VFP to be built when non-VFP capable CPUs are selected
Since we pass flags to the compiler to control code generation based
on the least capable selected CPU, if we want to include VFP support,
we must tweak the assembler flags to allow the VFP instructions.
Moreover, we must not use the mrrc/mcrr versions since these will not
be recognised by the assembler.

We do not convert all instructions to the VFP-equivalent (yet) since
binutils appears to barf on "fmrx rn, fpinst" and doesn't provide any
other way (other than using the mrc equivalent) to encode this
instruction - which is rather a problem when you have a VFP
implementation which requires these instructions.

d6551e884c [ARM] Add thread_notify infrastructure
Some machine classes need to allow VFP support to be built into the
kernel, but still allow the kernel to run even though VFP isn't
present. Unfortunately, the kernel hard-codes VFP instructions
into the thread switch, which prevents this being run-time selectable.

907d6deb62 [ARM] 3605/1: AT91RM9200 Power Management
Patch from Andrew Victor

This patch adds the core Power Management support for the AT91RM9200
processor. It will support suspend-to-RAM and standby modes.

The suspend-to-RAM functionality is not 100% complete. The code that
needs to be execute from the internal SRAM to restore the system is
outstanding. For now we just fall through to Standby mode.

The AT91-specific at91_suspend_entering_slow_clock() function will
eventually be replaced by clk_must_disable() once that functionality is
added to mainline clock API.

| We can kill them. I checked with Phil B. about them last week, and he

__NR_OABI_SYSCALL_BASE). In thumb mode syscall numbers start from zero.

The patch below fixes this by clearing the nigh bits of the syscall number
instead of inverting them. Technically this means we accept some invalid
syscall numbers, but I can't see how that could be a problem. The two sets of
numbers far apart that unimplemented syscalls should still be rejected.

Now that kconfig can load multiple configurations, it becomes simple to
integrate the split config step, by simply comparing the new .config file with

that this saves a bit of disk space and cache, as no data needs to be read

e2f04e1894 [ARM] 3546/1: PATCH: subtle lost interrupts bug on i.MX
Patch from Matt Reimer

There is a subtle bug in the GPIO interrupt status register
handling in arch/arm/mach-imx/irq.c:imx_gpio_ack_irq(). The
documentation states that a 1 should be written to the relevant bit to
acknowledge a GPIO interrupt, but that is not what the code does.

The problem is that the |= writes back 1s for all the *other*
interrupts represented in the register, so interrupts could get lost.
For example, if interrupts are pending for GPIO B10 and B12, ISR_B
would have the value 0x00001400. Then when the interrupt code handles
GPIO B10, it eventually calls imx_gpio_ack_irq(IRQ_GPIOB(10)), which
effectively does this:

ISR_B |= 1 << 10;

with the result that (0x00001400 | 0x00000400) is written, clearing
the interrupt status bits for *both* GPIO B10 and B12.

The fix is to write 1s only for the interrupts we want to clear.

The same problem seems to be occurring in the DMA code; this patch
does not address those issues.

d782f33df7 [ARM] Fix Neponset IRQ handling
While testing the genirq code on ARM, a condition was found whereby
the Neponset IRQ handler was being re-entered, causing the system
to deadlock.

Under the ARM IRQ code, this would not have been a visible problem
because the "simple" IRQ handling had no re-entrancy protection.

Resolve this by acknowledging the parent interrupt after we mask it
when we are going to handle one of our "special" level-based sources
(from ethernet or USAR chip.)

ec8510f6fe [ARM] 3540/1: ixp23xx: deal with gap in interrupt bitmasks
Patch from Lennert Buytenhek

On the ixp23xx, the microengine thread interrupt sources are numbered
56..119, but their mask/status bits are located in bit positions 64..127
in the various registers in the interrupt controller (bit positions
56..63 are unused.)

We don't deal with this, so currently, when asked to enable IRQ 64, we
will enable IRQ 56 instead.

The only interrupts >= 64 are the thread interrupt sources, and there
are no in-tree users of those yet, so this is fortunately not a big
problem, but this needs fixing anyway.

c3fb04162a [ARM] 3529/1: s3c24xx: fix restoring control register with undefined instruction
Patch from Dimitry Andric

In arch/arm/mach-s3c2410/sleep.S, the coprocessor registers are saved at
suspend time, and restored at resume time. However, an undefined
instruction is used when attempting to restore a non-existent "auxiliary
control register". This leads to a crash on S3C2412, which has an ARM926
core instead of an ARM920.

At suspend time, the following fragment runs:

	mrc	p15, 0, r7, c2, c0, 0	@ translation table base address
	mrc	p15, 0, r8, c2, c0, 0	@ auxiliary control register
	mrc	p15, 0, r9, c1, c0, 0	@ control register

and at resume time, the following fragment runs:

	mcr	p15, 0, r7, c2, c0, 0 @ translation table base
	mcr	p15, 0, r8, c1, c1, 0 @ auxilliary control
	...
	mcr	p15, 0, r9, c1, c0, 0 @ turn on MMU, etc

There are several problems with these fragments:
1. The ARM920 and ARM926 cores don't have any "auxiliary control
register", at least not according to the ARM920 and ARM926 TRM's.
2. The 2nd line of suspend erroneously saves the c2 register again.
3. This saved c2 value is restored using an undefined instruction. For
some reason this does not crash on ARM920, but does crash on ARM926.

The following patch fixes all these problems.

036999d501 [ARM] for_each_possible_cpu
for_each_cpu() actually iterates across all possible CPUs. We've had mistakes
in the past where people were using for_each_cpu() where they should have been
iterating across only online or present CPUs. This is inefficient and
possibly buggy.

We're renaming for_each_cpu() to for_each_possible_cpu() to avoid this in the
future.

This patch replaces for_each_cpu with for_each_possible_cpu.

6cc1b65886 [ARM] 3480/1: ixp4xx: fix irq2gpio array type
Patch from Lennert Buytenhek

The irq2gpio array was recently converted from an array of ints to an
array of chars (by patch 3368/1.) However, this array contains elements
that are -1, and on ARM, the char type is unsigned by default, so this
patch broke the GPIO check in ixp4xx_set_irq_type.

Change the 'char' to be a 'signed char' to fix this.

894b5779ce [PATCH] No arch-specific strpbrk implementations
While cleaning up parisc_ksyms.c earlier, I noticed that strpbrk wasn't
being exported from lib/string.c. Investigating further, I noticed a
changeset that removed its export and added it to _ksyms.c on a few more
architectures. The justification was that "other arches do it."

I think this is wrong, since no architecture currently defines
__HAVE_ARCH_STRPBRK, there's no reason for any of them to be exporting it
themselves. Therefore, consolidate the export to lib/string.c.

c80d79d746 [PATCH] Configurable NODES_SHIFT
Current implementations define NODES_SHIFT in include/asm-xxx/numnodes.h for
each arch. Its definition is sometimes configurable. Indeed, ia64 defines 5
NODES_SHIFT values in the current git tree. But it looks a bit messy.

SGI-SN2(ia64) system requires 1024 nodes, and the number of nodes already has
been changeable by config. Suitable node's number may be changed in the
future even if it is other architecture. So, I wrote configurable node's
number.

This patch set defines just default value for each arch which needs multi
nodes except ia64. But, it is easy to change to configurable if necessary.

On ia64 the number of nodes can be already configured in generic ia64 and SN2
config. But, NODES_SHIFT is defined for DIG64 and HP'S machine too. So, I
changed it so that all platforms can be configured via CONFIG_NODES_SHIFT. It
would be simpler.

1356c1948d [ARM] 3473/1: Use numbers 0-15 for the VFP double registers
Patch from Catalin Marinas

This patch changes the double registers numbering to 0-15 from even 0-30,
in preparation for future VFP extensions. It also fixes the VFP_REG_ZERO
bug (value 16 actually represents the 8th double register with the original
numbering).

FMRRD/FMDRR. The patch changes to CP11 for the correct instructions.

56ca904053 [ARM] 3457/1: i.MX: SD/MMC support for i.MX/MX1
Patch from Pavel Pisa

This patch adds support of i.MX/MX1 SD/MMC controller.
It has been significantly redesigned from the original Sascha Hauer's
version to support scatter-gather DMA, to conform to latest Pierre Ossman's
and Russell King's MMC-SD Linux 2.6.x infrastructure.
The handling of all events has been moved to the softirq context
and is designed with no busy-looping in mind. Unfortunately
some controller bugs has to be overcome by limited looping
about 2-20 usec but these are observed only for initial card
recognition phase.

There are still some missing/missed IRQs problems under heavy load.
Help of somebody with access to the full SDHC design information
is probably necessary.

Regenerated against 2.6.16-git-060402 to solve clash with other patches.
