8bfb1a10f2 kmap: remove kmap_atomic_to_page()

20b271dfe9 arch/kmap: define kmap_atomic_prot() for all arch's
To support kmap_atomic_prot(), all architectures need to support
protections passed to their kmap_atomic_high() function.  Pass protections
into kmap_atomic_high() and change the name to kmap_atomic_high_prot() to
match.

Then define kmap_atomic_prot() as a core function which calls
kmap_atomic_high_prot() when needed.

Finally, redefine kmap_atomic() as a wrapper of kmap_atomic_prot() with
the default kmap_prot exported by the architectures.

d8c25836fa arch/kmap: don't hard code kmap_prot values
To support kmap_atomic_prot() on all architectures each arch must support
protections passed in to them.

Change csky, mips, nds32 and xtensa to use their global constant kmap_prot
rather than a hard coded value which was equal.

abca2500c0 arch/kunmap_atomic: consolidate duplicate code
Every single architecture (including !CONFIG_HIGHMEM) calls...

	pagefault_enable();
	preempt_enable();

... before returning from __kunmap_atomic().  Lift this code into the
kunmap_atomic() macro.

While we are at it rename __kunmap_atomic() to kunmap_atomic_high() to
be consistent.

78b6d91ec7 arch/kmap_atomic: consolidate duplicate code
Every arch has the same code to ensure atomic operations and a check for
!HIGHMEM page.

Remove the duplicate code by defining a core kmap_atomic() which only
calls the arch specific kmap_atomic_high() when the page is high memory.

e23c45976f arch/kunmap: remove duplicate kunmap implementations
All architectures do exactly the same thing for kunmap(); remove all the
duplicate definitions and lift the call to the core.

This also has the benefit of changing kmap_unmap() on a number of
architectures to be an inline call rather than an actual function.

525aaf9bad arch/kmap: remove redundant arch specific kmaps
The kmap code for all the architectures is almost 100% identical.

Lift the common code to the core.  Use ARCH_HAS_KMAP_FLUSH_TLB to indicate
if an arch defines kmap_flush_tlb() and call if if needed.

This also has the benefit of changing kmap() on a number of architectures
to be an inline call rather than an actual function.

01c4b788e0 arch/kmap: remove BUG_ON()
Patch series "Remove duplicated kmap code", v3.

The kmap infrastructure has been copied almost verbatim to every
architecture.  This series consolidates obvious duplicated code by
defining core functions which call into the architectures only when
needed.

Some of the k[un]map_atomic() implementations have some similarities but
the similarities were not sufficient to warrant further changes.

In addition we remove a duplicate implementation of kmap() in DRM.

This patch (of 15):

Replace the use of BUG_ON(in_interrupt()) in the kmap() and kunmap() in
favor of might_sleep().

Besides the benefits of might_sleep(), this normalizes the implementations
such that they can be made generic in subsequent patches.

5093c5872b mm/vma: append unlikely() while testing VMA access permissions
It is unlikely that an inaccessible VMA without required permission flags
will get a page fault.  Hence lets just append unlikely() directive to
such checks in order to improve performance while also standardizing it
across various platforms.

33e53ae1ce csky: Add kprobes supported
This patch enable kprobes, kretprobes, ftrace interface. It utilized
software breakpoint and single step debug exceptions, instructions
simulation on csky.

We use USR_BKPT replace origin instruction, and the kprobe handler
prepares an excutable memory slot for out-of-line execution with a
copy of the original instruction being probed. Most of instructions
could be executed by single-step, but some instructions need origin
pc value to execute and we need software simulate these instructions.

dd7c983e78 csky/ftrace: Fixup ftrace_modify_code deadlock without CPU_HAS_ICACHE_INS
If ICACHE_INS is not supported, we use IPI to sync icache on each
core. But ftrace_modify_code is called from stop_machine from default
implementation of arch_ftrace_update_code and stop_machine callback
is irq_disabled. When you call ipi with irq_disabled, a deadlock will
happen.

We couldn't use icache_flush with irq_disabled, but startup make_nop
is specific case and it needn't ipi other cores.

359ae00d12 csky: Fixup ftrace modify panic
During ftrace init, linux will replace all function prologues
(call_mcout) with nops, but it need flush_dcache and
invalidate_icache to make it work. So flush_cache functions
couldn't be nested called by ftrace framework.

Some CPUs don't support icache.va instruction to maintain the whole
smp cores' icache. Using icache.all + IPI casue a lot on performace
and using defer mechanism could reduce the number of calling icache
_flush_all functions.

7f4a567332 csky/mm: Fixup export invalid_pte_table symbol
There is no present bit in csky pmd hardware, so we need to prepare invalid_pte_table
for empty pmd entry and the functions (pmd_none & pmd_present) in pgtable.h need
invalid_pte_talbe to get result. If a module use these functions, we need export the
symbol for it.

f525bb2c9e csky: Tightly-Coupled Memory or Sram support
The implementation are not only used by TCM but also used by sram on
SOC bus. It follow existed linux tcm software interface, so that old
tcm application codes could be re-used directly.

Software interface list in asm/tcm.h:
 - Variables/Const: 	__tcmdata, __tcmconst
 - Functions:		__tcmfunc, __tcmlocalfunc
 - Malloc/Free:		tcm_alloc, tcm_free

In linux menuconfig:
 - Choose a TCM contain instrctions + data or separated in ITCM/DTCM.

8e3a68fb55 dma-mapping: make dma_atomic_pool_init self-contained
The memory allocated for the atomic pool needs to have the same
mapping attributes that we use for remapping, so use
pgprot_dmacoherent instead of open coding it.  Also deduct a
suitable zone to allocate the memory from based on the presence
of the DMA zones.

ae76f635d4 csky: Optimize arch_sync_dma_for_cpu/device with dma_inv_range
DMA_FROM_DEVICE only need to read dma data of memory into CPU cache,
so there is no need to clear cache before. Also clear + inv for
DMA_FROM_DEVICE won't cause problem, because the memory range for dma
won't be touched by software during dma working.

Changes for V2:
 - Remove clr cache and ignore the DMA_TO_DEVICE in _for_cpu.
 - Change inv to wbinv cache with DMA_FROM_DEVICE in _for_device.

memory range, cache_op will panic in do_page_fault with bad_area.

This bug is from commit: 2b070ccdf8 (fixup abiv2 mmap(... O_SYNC)
failed). In that patch we remove the _PAGE_SO for memory noncache
mapping and this will cause problem when drivers use dma descriptors
to control the transcations without dma_w/rmb().

After referencing other archs' implementation, pgprot_writecombine is
introduced for mmap(... O_SYNC).

There are two generations of tlb operation instruction for C-SKY.
First generation is use mcr register and it need software do more
things, second generation is use specific instructions, eg:
 tlbi.va, tlbi.vas, tlbi.alls

We implemented the following functions:

 - flush_tlb_range (a range of entries)
 - flush_tlb_page (one entry)

 Above functions use asid from vma->mm to invalid tlb entries and
 we could use tlbi.vas instruction for newest generation csky cpu.

 - flush_tlb_kernel_range
 - flush_tlb_one

 Above functions don't care asid and it invalid the tlb entries only
 with vpn and we could use tlbi.vaas instruction for newest generat-
 ion csky cpu.

a231b8839c csky: Add new asid lib code from arm
This patch only contains asid help code from arm for next patch to
use.

The asid allocator use five level check to reduce the cost of
switch_mm.

 1. Check if the asid version is the same (it's general)
 2. Check reserved_asid which is set in rollover flush_context()
    and key point is to keep the same bit position with the current
    asid version instead of input version.
 3. Check if the position of bitmap is free then it could be set &
    used directly.
 4. find_next_zero_bit() (a little performance cost)
 5. flush_context  (this is the worst cost with increase current asid
    version)

Check is level by level and cost is also higher with the next level.
The reserved_asid and bitmap mechanism prevent unnecessary
find_next_zero_bit().

The atomic 64 bit asid is also suitable for 32-bit system and it
won't cost a lot in 1th 2th 3th level check.

The operation of set/clear mm_cpumask was removed in arm64 compared to
arm32. It seems no side effect on current arm64 system, but from
software meaning it's wrong. Although csky also needn't it, we add it
back for csky.

The asid_per_ctxt is no use for csky and it reserves the lowest bits for
other use, maybe: trust zone ? Ok, just keep it in csky copy.

Seems it also could be used by other archs and it's worth to move asid
code to generic in future.

9d35dc3006 csky: Revert mmu ASID mechanism
Current C-SKY ASID mechanism is from mips and it doesn't work well
with multi-cores. ASID per core mechanism is not suitable for C-SKY
SMP tlb maintain operations, eg: tlbi.vas need share the same asid
in all processors and it'll invalid the tlb entry in all cores with
the same asid.

This patch is prepare for new ASID mechanism.

  fault (such as a page fault) and the kernel responded with a signal.

  Because the name force_sig does not make this clear, and because the
  force_sig takes a task parameter the function force_sig has been
  abused for sending other kinds of signals over the years. Slowly those
  have been fixed when the oopses have been tracked down.

  This set of changes fixes the remaining abusers of force_sig and
  carefully rips out the task parameter from force_sig and friends
  making this kind of error almost impossible in the future"

2e1661d267 signal: Remove the task parameter from force_sig_fault
As synchronous exceptions really only make sense against the current
task (otherwise how are you synchronous) remove the task parameter
from from force_sig_fault to make it explicit that is what is going
on.

The two known exceptions that deliver a synchronous exception to a
stopped ptraced task have already been changed to
force_sig_fault_to_task.

The callers have been changed with the following emacs regular expression
(with obvious variations on the architectures that take more arguments)
to avoid typos:

8a7f97b902 treewide: add checks for the return value of memblock_alloc*()
Add check for the return value of memblock_alloc*() functions and call
panic() in case of error.  The panic message repeats the one used by
panicing memblock allocators with adjustment of parameters to include
only relevant ones.

The replacement was mostly automated with semantic patches like the one
below with manual massaging of format strings.

2b070ccdf8 csky: fixup abiv2 mmap(... O_SYNC) failed.
Glibc function mmap(... O_SYNC) will make page to _PAGE_UNCACHE +
_PAGE_SO and strong-order page couldn't support unalignment access.
So remove _PAGE_SO from _PAGE_UNCACHE, also sync abiv1 with the macro
of _PAGE_SO.
